How does a transition coordinator work? What is important to understand about youths who are about to take responsibility for their own care? Within the research project Stepstones, the crucial transition from pediatric to adult healthcare for patients with chronic conditions such as type one diabetes and congenital heart defects is studied. Now, researchers have developed a series of films that describe this important process, which requires preparation, participation, and self-determination. These films showcase different perspectives of the evidence-based transition programs currently being tested and evaluated at various hospitals in Sweden.
